thoroughly confused about tax forms

I had Sam in Oct. 2010. I returned to work towards the end of January 2011.

My office's vp admin left a few months ago, and his replacement started today and we have no HR dept, so i have no clue who else to ask.

I am pretty sure that my office paid for 6 weeks maternity leave, full pay. Those weeks were in 2010. For 6 weeks from mid-Dec 2010 through mid-to-late-January 2011,  I am pretty sure I used sick and vacation time for about 3 weeks of full pay and then there were 3 weeks of STD to cover some more pay.

My husband thinks I need Form 1099-G for unemployment or paid family leave. I don't think the STD is part of that - is it? Paid family leave was covered in 2010, but we have no clue about the three weeks in 2011....I can ask our company boss to look into what's missing with our payroll service, but I want to be clear in what I need first.
