Leah at Cutesie Blog Design did mine.  She was incredibly easy to work with and the turn around time was pretty quick.  Her pricing is very reasonable as well.

Shay Bocks did mine.  If you scroll down to the bottom of my blog's main page, there's a link, or you can just google it.  She was great to work with.  Kinda pricy though.

Erin of Samantha's Day blog design designed mine and I love it.  However, she did it last year and the same package this year is 4 x the amount I spent on it last year!

I used a template from Suck My Lolly (crazy name, I know).  The templates are inexpensive and she was very nice about any questions that I had. You still do most of the work with installing and adding words to the header but it was worth $10.
