explain to me "optional short term disability" and other benefits questions

if a company offers "optional short term disability" how does that typically work?  Is it something that I contribute to out of my paycheck in case I need it, or does the company in some way typically match?  I'm interviewing for a job at a new company and their website lists that under their benefits.  Right now at my current job std is included and i've been there long enough for it to cover my salary 100% while on it.  I'm not sure if we're done having kids so this particular benefit is one I really have interest in.

At what point in the interviewing process is it appropriate to ask about this?  How about things such as company matching 401k or details on health insurance such as how much the employee pays and what the coverage is?
