For those that DIY, where do you get the top part (crochet band)? I looked at Hobby Lobby so far and they do not have anything wide enough (6" + wide). I saw that I can get them online, but I would end up paying more in shipping than the band.

I order them online, but I order a lot of them at once (well, my MIL does), so it negates the extra cost when you break it down.
Can you connect two of the skinnier ones together somehow?

I've seen so many different ideas for how to make tutus so this may be different from what you had in mind, but... When I made DDs I started with a 4 or 5" tall rectangle that was wide enough to just barely wrap around her body. It was a comfortable cotton so that the actual tulle doesn't touch her body. I sewed all of the tulle onto that one row at a time pleating it and sewing it upside down starting from the bottom and ending on the top layer. At the very end, when all of the cotton (except maybe 1/2") was covered in the tulle layers, I sewed a satin ribbon on the top with just a simple machine stitch at the top and bottom and left a tail on both sides to tie it with. Not sure if that makes sense, but maybe it'll inspire you to not need the piece you're thinking of?
If you're doing a no-sew tutu with an elastic waist band, could you just buy plain elastic from the fabric store and sew or tie (depending on how thick it is) the ends together?
