Care for kids while out of the country

We booked a trip to Dominican Republic in January and it will be our first big trip away from our children. The grandparents will be watching the kids. What do I need to do to ensure that they will be able to care for the kids while we are not in the country? Do I need something notorized so they could be responsible in a hospital/doctor setting?

  • Our doctor had a form that we filled out that allowed our parents to get medical attention for our kids (and our son actually got sick while we were in Asia and had to go to the doctor). We also had a letter we wrote allowing them to get treatment by our parents at a hospital. Leave your insurance cards with them.

  • We left our DD overnight for the first time recently to go to a friend's wedding in Cancun. 

    Here are some things we did:

    Made sure our wills were up to date

    Drafted a letter saying that they could make emergency medical decisions on our behalf.   We signed it and notarized it (my friend's mom is a notary though, and did it for free for us)

    Left a copy of DD's insurance card, along with the number to her pedi

    Made a list of food ideas, along with a general schedule

    Left medicines out, just in case, along with proper dosage info for her weight (which we also wrote down)

    Left all of our flight/hotel information

  • We've traveled without our kids several times. Here's what we do.

    We have a permanent medical authorization form for our parents with each child's complete medical history and relevant family history that was both notarized and witnessed, in addition to health insurance, dental insurance, and vision insurance cards. We also left copies of our drivers licenses in case they were needed for records at a hospital or anything.

    We always make sure our wills are up to date and that someone knows how to access them, just in case.

    We have an emergency contact sheet that has every phone number they could ever need: pediatrician, poison control, nurse help line, hotels & airlines, other family members, etc. It sounds silly, but it an emergency I want them to have it all on one sheet. They just keep it on the fridge.

    We leave them medications with doses written out for them.  

    We also leave them a folder with copies of our passports, and all of our travel arrangements/tickets. If we ever lost them, we could have them fax or email them, and then if they lose our info sheet, they still know how to get ahold of us.
