  I can definitely fax the form. If I find it in my house tonight, I'm faxing it tomorrow. But if I have to wait for the second package to arrive tomorrow, I won't get it until I get home tomorrow and then I can't get it signed until Monday.

  Yeah I would see if you could fax it or email.  We did that for DH's FMLA leave when I was pregnant.  The doctor's office seemed to have dealt with stuff like that all the time.

  We finally had some money saved up thanks to taxes. 
  Last night's wind storm broke a tree.  It's leaning against another tree and is about to fall on a neighbor's shed.  Great.

  I saw this today at the DMV. You can't tell, because the pic is blurry, but this woman is at least 50 years old. Or, she's 25 with a really bad drug problem. Those aren't pants. They're boots that go up to her thigh. 4" heels. Leather. I'll let the photo tell the rest.
